How to Register
- Fill out the registration form online or call us and we can register you on the phone.
- Download and fill out the Liability Form.
- Include a copy of each participant's health insurance card.
- Payment can be made online, by check, or credit card. We prefer one check or credit card payment per group.
- Chaperones need the chaperone form completed by your pastor and Safe Environment certification (or diocese equivalent).
